When you see Dr. Cleaveland, you will pay for the session at the time of service (see the paperwork for the fees). You may call your insurance company to find out how much will be covered. The questions on the insurance worksheet will help you understand what they will cover. You may also want to read your insurance booklet under "behavioral health" or "mental health" to better understand your benefits. If you have Medicare: Dr. Cleaveland has opted out of Medicare. Neither Dr. Cleaveland NOR you can file your sessions with Medicare, so you will pay the full fee out of pocket without reimbursement. You may wish to find an in network provider, where your out-of-pocket expenses will be much lower. |
